Care Management is performed by degreed, certified professionals who act as advocates for the elderly or disabled. By evaluating an individual's level of functioning, their health, their emotional state, their finances and their legal needs, a Care Manager helps to identify problems and offers timely, practical solutions.

A Care Manager can help ease the stress of searching for Long Term Care solutions by:

  • Offering counseling, support and information
  • Advocating on your behalf
  • Intervening in a crisis

The Care Manager works with their clients, their families, senior communities, trust officers, allied health professionals, attorneys and guardians to assure that their client’s needs are met at the highest level thereby providing the highest quality of life.

A Care Manager can be hired to:

  • Help out in a time of crisis,
  • Should you need a respite
  • As the full time point of contact for all senior services.

Client advocacy

  • Assure that clients are treated with respect and dignity from all providers in all situations
  • Assure that medical needs are met
  • Accompany clients to appointments when needed
  • Explain medical diagnoses and treatments
  • Counsel families and care-givers
  • Assist with long-term health care planning and appropriate placement if needed
  • Assure that personal, social and homemaking needs are maintained in-home or in a care facility
  • Client and family support

Care Management

  • 24 Hour, Seven day availability
  • Periodic skilled observation and assessment
  • Develop and implement a plan of care
  • Arrange appointments and assure they are kept
  • Instruct clients and care-givers in disease management, diet, treatments and medication
  • Manage and monitor medication
  • Arrange for community services and home subcontractors
  • Arrange for and oversee care- givers, housekeeping, transportation and bill paying

Case Coordination

  • Keep concerned families informed (in-town and out of state)
  • Report to guardians, attorneys or trust officers
  • Economically and efficiently coordinate services
  • Coordinate care as directed by other professionals
  • Avoid duplication of services
  • Consult with and refer other professionals to maintain current documentation
    of medical and end of life desires
  • Review insurance policies and work with insurance companies to obtain
    benefits.
